Margera’s departure from the rehab facility was first reported by TMZ.
Margera achieved fame in the early 2000s as one of the stars of the MTV reality show “Jackass” and its subsequent spinoffs.
According to the police report, Margera said he was going to check himself into another Delray Beach facility, WPTV reported. The manager of the first rehab center told police he saw the pro skateboarder leave in a black Honda sedan.
Officer Jonathan Belman Otero wrote that he went to the other facility to look for Margera, but he was not there, according to the television station.
In May, Margera wrote in an Instagram post that he had been sober for one year.
